Onboarding Forms
Patient authorizations are required for Hometown Specialty Services through CMS guidelines and the URAC and ACHC specialty pharmacy standards of care for every individual patient to be provided appropriate onboarding paperwork and consent.
REMs Medication Forms
Risk Evaluation and Mitigation Strategy (REMS) is an FDA program ensuring a medication’s benefits outweigh its risks.While prescribers do not need REMS certification for Brixadi and Sublocade, we provide these onboarding forms to simplify compliance for new providers. Send us the prescription prior to the patient’s appointment, and our certified REMS pharmacy will coordinate delivery directly to your clinic.
Note: Brixadi and Sublocade are never dispensed directly to patients and must be administered by a healthcare professional in a clinical setting.
