Germaine Dunn, community health worker

Germaine Dunn is a native of Harrodsburg, KY. He grew up in a Christian environment, he enjoyed playing sports and being creative. Germaine graduated from Harrodsburg Highschool and unfortunately over many years found himself overcome through a life of addiction, homelessness and incarceration.

His recovery journey

After years of struggling with addiction, Germaine walked through the doors of Isaiah House at age 43, changing his life forever. Within a year of sobriety, he restored his family relationships and received full custody of his youngest child. He went on to graduate Dean's List with a BFA from Campbellsville University, earning top alumni awards along the way.

What he does now

Germaine has dedicated his career to recovery, serving as an art teacher, peer support specialist, and community health worker. Today, he works with Hometown Specialty in Campbellsville, KY, drawing on his lived experience with addiction and homelessness to connect others with life-changing resources. Recently married, Germaine focuses every day on family, faith, and offering hope to those on their own recovery journey.

Upon admission, we conduct a screening to identify barriers to care. We assess needs related to housing, transportation, food security, medication affordability, and personal support systems to connect you with the right resources.

Advocacy

Wrap-around support system

Every client is paired with a peer support specialist and a Patient Care Advocate. Our team coordinates across pharmacy, providers, and care facilities to manage your benefits and support your overall goals.

Continuity

Transition of care planning

Navigating multiple providers can lead to gaps in care during transitions. Our integrated team advocates for you, ensuring your health records and treatment plans transfer seamlessly between facilities and providers.

Hometown Connect enrollment

We connect clients with top providers, non-profits, and specialized community resources. We believe everyone deserves whole-person healing and the proper support to live independently and thrive.

Access

Financial hardship programs

Our Integrated Care Team, Patient Care Advocates, and Doctors of Pharmacy collaborate directly with your healthcare providers and drug manufacturers to ensure your specialty treatments are both affordable and accessible. If you are experiencing financial difficulties, income changes due to a diagnosis, or fall below the Federal Poverty Level, you may qualify for aid through the Hometown Pharmacy Hardship Program.
